Forms may be filled out prior to your first visit and you may need a consultation prior to start of treatment. Your prescribed medications may be shipped to your home address. All initial evaluations must be completed in person.

Initial Office/Physician Visit = $150.00

Follow-up consultations = $75.00

Patients have the option of obtaining prescribed medication (generic or brand) from our on-premises dispensary.

CPRx's Outpatient Detoxification Programs are designed to end the patient's mental and/or physical dependence or tolerance to opiates such as heroin, codeine, hydrocodone, morphine and opiate derivatives sold under a variety of tradenames.

Program costs are related to the amount of medication and office visits required to properly detoxify.

Payments may be made with a credit card, debit card, personal check or cash. Health insurance is not accepted for reimbursement of services, however, CPRx will prepare documentation for a patient to present to their insurance company for those patients whose insurance will cover treatment.

Some patients require ongoing pain management and/or medication management after completion of the initial program. Physician's fees follow the schedule above. Patients have the option of obtaining prescribed medication at our on-premises dispensary.
